Background Air4ce Kids, Air4ce Young Adults and Care4ces
Air4ce Kids is a relatively new, unique children’s asthma center in the region and part of the cooperative association Care4ces. Care4ces has set itself the goal of shaping care together with the patient. In concrete terms, this means that our care focuses on the personal situation of the child and his or her family. We assume that the family itself is most familiar with its own situation, wishes and preferences.
Because the referrers asked us to expand our care to young adults, we have now also launched Air4ce Young Adults. It appears that there are many questions about asthma in the group of young adults (18-26 years). Because asthma in young adults has many similarities with asthma in children, we thought we could add this group. Here too, the emphasis is on the personal situation and the personal wishes and preferences.
For us, collaboration and learning from each other are also the most important aspects in the relationship with you as a referrer.
Referring a child or young adult to Air4ce Kids/Young Adults
In principle, all children and young adults with respiratory complaints can be referred to us. In recent years, we have specialized in diagnosing and guiding children and young people with asthma. Experience shows that making this diagnosis is not easy, especially not in (young) children.
Reimbursement by health insurers
The care of Air4ce Kids & Young Adults is reimbursed. The care for children and young people up to 18 years old falls outside the deductible of the parents or guardians.
We currently have a care contract with Zorg&Zekerheid. We have a payment agreement with a number of other health insurers. As part of our goodwill policy, we have ensured in all cases that parents and young adults do NOT have to pay a personal contribution if 100% is not reimbursed (18+ so only the usual deductible).
PLEASE NOTE: RMA/RMO unfortunately does not reimburse care with us, not even partially. Patients who are insured with this health insurer are best referred to a hospital.
Referring a child
If you suspect that a child or young adult may have asthma, there are several possibilities:
- Consultation
- Referral for diagnosis and advice
- Diagnosis and transfer of treatment
Consultation
We are always prepared to consult. Whether it concerns general questions about asthma or related complaints, diagnosis and treatment, or specific advice about children/young adults who are or are not in our care. We value good contact, short lines, cooperation and learning from each other.
Referral for diagnosis and advice
Diagnosing asthma can be difficult. If you want to use our expertise, you can refer the child/young adult for diagnosis and associated treatment advice. We are careful in diagnosing and advising. We often need some time for this. We have the facilities to perform various examinations and work closely with, for example, the SCAL and various (child) physiotherapists. X-rays can also be realized in collaboration with the hospitals. In this way, we can facilitate all possible facets of the diagnostic process.
After the process, we will discuss the results with the child and parents or caregivers or with the young adult. And of course, we will report this back to you. You can indicate in advance whether you would like personal contact or would prefer to be informed by email or letter. If, in our opinion, the situation requires personal contact with each other, we will let you know.
Diagnosis and transfer of treatment
You may have a child/young adult in the practice for whom you would prefer us to take on the entire care from diagnosis to treatment. Then we can provide that care. This can be for a shorter or longer period. Here too, it goes without saying that you can indicate your preferences. If we assess the situation differently, we will consult with you.
Refer back or refer to others
If it appears that other care is required, for example general pediatric care, we will refer the child back to you in consultation with the child and parents or caregivers, or if necessary/desired, refer them directly to the relevant healthcare provider.
Registration procedure
Referring a patient can be done easily via the healthcare domain, but of course also by means of a paper referral letter or via healthcare email.
There are several options for registering a patient:
- via Zorgdomein (preferred) : we will then contact the patient or parents as soon as possible
- by phone: 085 82 24 810
- via the parents/patient: they can register their child via the contact form on our website. We will then create a patient portal in which they can upload your referral letter
- via care mail: care4cesua@zorgmail.nl
We are not always able to answer the phone immediately, but if you leave your contact details we will contact you as soon as possible.
Information material
