This privacy statement applies to the processing of personal data by Care4ces UA and Air4ce Kids & Young Adults, with registered office in Leiden and offices at Dieperpoellaan 28 E Leiden, registered in the register of the Chamber of Commerce under number 74172603.

In this document we provide information about how we handle your data or that of your child.

Naturally, we attach great importance to the protection of your personal data and that of your child. The data is always treated confidentially and in accordance with privacy legislation (General Data Protection Regulation, GDPR).

The data is exchanged via a secure connection between the Electronic Patient Records (EPDs) and/or through an electronic exchange system.

Every healthcare provider has a duty of confidentiality. You may therefore expect us to always handle your data or that of your child with care.

Unless otherwise agreed, we will only report to the person who referred you/your child (usually the family doctor or a representative thereof) and up to the age of 18 to the youth doctor of the Youth and Family Center (“the consultation bureau”). If you object to this, you can indicate this and we will of course not exchange the data.

Health insurers, employers or company doctors may not request or consult medical data without your permission.

Medical data of children are stored indefinitely (in principle 110 years after the date of birth). At your request, or depending on the age at the request of your parents, we can destroy the data earlier. In exceptional cases, the request for destruction can be refused.

Medical data of adults (18 years and older) are required to be retained for 20 years after completion of care. Once this period has expired, the data will be destroyed. Under certain circumstances, the data will be retained for a longer period: if this is necessary for good care, if this is in the interest of another person or if there is a legal obligation to do so.

Parents have the right to view the medical records of their children up to the age of 12. To view the records of a child between the ages of 12 and 16, the child’s permission is required, unless parents need the information to give permission for treatment. From the age of 16, young people have the right to view their own records, and of course parents may also view the records from the age of 16, but only with the young person’s permission.
